  1. SPONSOR

This Sweepstakes (the “Sweepstakes”) is sponsored by Van Scoy Hair Clinic (“Sponsor”), with clinic locations in Parma, Columbus, and Ashland, Ohio. All questions, comments, or complaints regarding this Sweepstakes should be directed to the Sponsor and not to any advertising or promotional agency.

  1. ELIGIBILITY

The Sweepstakes is open only to natural persons who are:

(a)  Legal residents of the State of Ohio;

(b)  Eighteen (18) years of age or older at the time of entry; and

(c)  Not otherwise disqualified under these Official Rules.

The following individuals are not eligible to participate: employees, officers, directors, contractors, agents, and representatives of the Sponsor, and their immediate family members (defined as spouse, parent, child, sibling, and their respective spouses), and any persons residing in the same household as such individuals, whether or not related.

Participation in this Sweepstakes constitutes entrant’s full, unconditional, and irrevocable agreement to these Official Rules and to the decisions of the Sponsor, which are final and binding in all matters related to this Sweepstakes.

  1. SWEEPSTAKES ENTRY PERIOD

The Sweepstakes begins on June 15, 2026 at 12:00 a.m. Eastern Time (“ET”) and ends on June 29, 2026 at 11:59 p.m. ET (the “Entry Period”). Entries submitted before or after the Entry Period will not be considered. Sponsor’s computer or other timekeeping device is the official clock for the Sweepstakes.

  1. HOW TO ENTER

During the Entry Period, eligible individuals may enter the Sweepstakes as follows:

ONLINE ENTRY (NO PURCHASE NECESSARY): Visit https://www.vanscoyhair.com/register and complete and submit the online entry form with all required information, including your full legal name, valid email address, and telephone number. Limit one (1) entry per person and per email address for the duration of the Entry Period.

Entries generated by script, macro, bot, or other automated or programmatic means will be disqualified. Entries that are incomplete, illegible, corrupted, lost, misdirected, or otherwise not in compliance with these Official Rules will be void and will not be accepted. Proof of submission does not constitute proof of receipt. Sponsor is not responsible for technical, network, telephone, computer hardware or software failures of any kind, lost or unavailable network connections, or failed, incomplete, garbled, or delayed computer transmissions that may limit an entrant’s ability to participate.

  1. PRIZE AND APPROXIMATE RETAIL VALUE

Grand Prize (1): One (1) six-month PRP (Platelet-Rich Plasma) Hair Restoration Treatment Package provided by Van Scoy Hair Clinic.

Approximate Retail Value (“ARV”): $3,500.00

The prize consists solely of professional hair restoration services as determined and provided by Van Scoy Hair Clinic. The prize is not a cash award and has no cash equivalent. No cash redemption, cash substitution, transfer, or assignment of the prize by the winner is permitted except at Sponsor’s sole discretion or as otherwise required by applicable law.

IMPORTANT: MEDICAL ELIGIBILITY AND PRIZE SUBSTITUTION:

Prior to receiving any treatment, the winner must complete a clinical consultation and medical evaluation conducted by Van Scoy Hair Clinic to assess the winner’s suitability for PRP hair restoration treatment. If, in the sole and professional judgment of Van Scoy Hair Clinic, PRP treatment is not medically appropriate, medically advisable, clinically suitable, or otherwise not recommended for the winner based on the results of the clinical consultation and medical evaluation (including, without limitation, where the degree of hair loss is determined to be too advanced for PRP to be effective), the Sponsor reserves the right, in its sole discretion, to substitute all or any portion of the prize with alternative hair restoration treatments, services, products, or clinic credits of comparable value. Any substituted prize shall be determined solely by the Sponsor based upon the winner’s individual treatment needs and medical suitability as assessed at consultation. The winner shall have no right to any specific treatment modality or to any cash equivalent in lieu of services.

Only one (1) prize will be awarded. The prize is non-transferable. Sponsor is not responsible for any costs or expenses associated with prize acceptance or use not expressly described herein.

  1. WINNER SELECTION AND NOTIFICATION

On or about June 30, 2026, Sponsor (or its designated representative) will conduct a single random drawing from all eligible entries received during the Entry Period. The odds of winning depend on the total number of eligible entries received.

The potential winner will be notified by email and/or telephone using the contact information provided at the time of entry. If the potential winner cannot be contacted within five (5) business days of the first notification attempt, or if the potential winner fails to respond within five (5) business days after notification, the potential winner may be disqualified and an alternate winner may be selected from the remaining eligible entries at Sponsor’s sole discretion.

The prize will be awarded only if the potential winner timely responds to the notification and satisfies all requirements set forth in these Official Rules.

  1. WINNER REQUIREMENTS

As a condition of receiving the prize, the selected winner may be required to sign and return, within the time period specified by Sponsor:

(a)  An Affidavit of Eligibility confirming compliance with these Official Rules;

(b)  A Liability Release releasing Sponsor and its affiliates, officers, employees, directors, and agents from any and all liability arising out of or related to the Sweepstakes or the acceptance, use, or misuse of the prize;

(c)  A Publicity Release authorizing Sponsor to use the winner’s name, likeness, voice, photograph, and/or biographical information for advertising and promotional purposes without further compensation, where permitted by law; and

(d)  Any medical consent, intake, or acknowledgment forms required by Van Scoy Hair Clinic in connection with the clinical consultation and any treatment rendered.

If the winner fails to return any required documentation within the time specified, declines the prize, is found to be ineligible, does not comply with these Official Rules, or if the prize notification is returned as undeliverable, the winner will be disqualified and an alternate winner may be selected.

  1. PRIVACY

Personal information collected from entrants in connection with this Sweepstakes will be used by Sponsor for the purpose of administering the Sweepstakes and, if applicable, awarding the prize. Sponsor may also use entrant information for promotional, marketing, and internal business purposes in accordance with its Privacy Policy available at https://www.vanscoyhair.com/privacy-policy.

  1. GENERAL CONDITIONS AND SPONSOR’S RIGHT TO MODIFY OR CANCEL

Sponsor reserves the right, in its sole discretion, to cancel, terminate, modify, extend, or suspend this Sweepstakes, in whole or in part, at any time and for any reason, including but not limited to if fraud, technical failures, unauthorized intervention, or any other cause beyond Sponsor’s reasonable control corrupts or adversely affects the administration, security, fairness, integrity, or proper conduct of the Sweepstakes. In such event, Sponsor reserves the right to select a winner from among all eligible entries received prior to the action requiring such cancellation, termination, modification, extension, or suspension.

Sponsor reserves the right to disqualify any individual found to be: (a) tampering with the entry process or operation of the Sweepstakes; (b) acting in an unsportsmanlike or disruptive manner; or (c) violating these Official Rules. Any attempt by any person to deliberately damage any website or undermine the legitimate operation of this Sweepstakes may be a violation of criminal and civil law, and Sponsor reserves the right to seek damages and other remedies from any such person to the fullest extent permitted by law.

  1. RELEASE OF LIABILITY

By participating in this Sweepstakes, each entrant agrees to release, discharge, and hold harmless Sponsor and its respective parent companies, subsidiaries, affiliates, officers, directors, employees, agents, contractors, advertising and promotional agencies, and all other entities associated with the development and execution of this Sweepstakes (collectively, “Released Parties”) from any and all claims, demands, liability, damages, losses, or causes of action arising out of or related to: (a) participation in this Sweepstakes; (b) the acceptance, possession, use, or misuse of any prize; or (c) any typographical, printing, or other errors in any materials associated with the Sweepstakes.

RELEASED PARTIES ARE NOT RESPONSIBLE FOR: (i) incomplete, late, misdirected, damaged, or illegible entries; (ii) telephone, electronic, hardware, software, network, internet, computer, or other technical malfunctions, errors, or difficulties; (iii) any error, omission, interruption, defect, or delay in transmission; (iv) any injury or damage to persons or property; or (v) any other error or problem arising from participation in this Sweepstakes.

  1. DISPUTES AND GOVERNING LAW

This Sweepstakes is governed by and construed in accordance with the laws of the State of Ohio, without regard to its conflict of laws principles. Any and all disputes, claims, or controversies arising out of or relating to this Sweepstakes, the prize, or these Official Rules shall be resolved exclusively in the state courts of competent jurisdiction sitting in Cuyahoga County, Ohio, and each entrant irrevocably consents to the personal jurisdiction and venue of such courts and waives any objection to the laying of venue in Cuyahoga County, Ohio. To the extent permitted by law, each entrant waives any and all rights to a trial by jury in any such action.

Any and all disputes shall be resolved individually, without resort to any form of class action. In any dispute, an entrant shall not be entitled to obtain, and shall waive, any right to: (a) punitive, incidental, special, consequential, or other damages; (b) injunctive, declaratory, or other equitable relief; or (c) attorney’s fees, other than actual out-of-pocket costs incurred by the entrant.

  1. SEVERABILITY

If any provision of these Official Rules is held to be invalid, illegal, or unenforceable under applicable law, such provision shall be modified to the minimum extent necessary to make it enforceable, or if modification is not possible, shall be severed from these Official Rules, and the remaining provisions shall continue in full force and effect. The invalidity or unenforceability of any provision shall not affect the validity or enforceability of any other provision.

  1. WINNER’S LIST

To obtain the name of the prize winner (available after July 14, 2026), send a self-addressed, stamped envelope to: Van Scoy Hair Clinic, Attn: Father’s Day Giveaway Winner, 5592 Broadview Road; Parma, Ohio 44134. Requests must be received no later than August 30, 2026.
